Making Changes to or Requesting a Replacement Exempt Organization Certificate (Form ST-5)

Please only submit the form once (either by email, fax, mail or upload) to avoid a processing delay.

If you need a copy of your Form ST-5 or need to update the organization's name or physical location or mailing address, follow the steps below. If you need to make any other business registration changes for the organization, contact the ³Ô¹ÏºÚÁÏ Division of Revenue and Enterprise Services (DORES).

Replacement or Duplicate Forms

If you need a replacement or duplicate Form St-5, complete the ³Ô¹ÏºÚÁÏ Exempt Organization Request for Updated or Replacement ST-5 form . An organization officer must sign the form and include the tax exemption number.

Name Changes

For name changes, complete the ³Ô¹ÏºÚÁÏ Exempt Organization Request for Updated or Replacement ST-5 form . In addition, you must include a copy of the amendment to the certificate of incorporation or constitution with your request.

Address Updates

If you need to update the organizations' physical location and the organization is not registered for any other ³Ô¹ÏºÚÁÏ tax types, complete the ³Ô¹ÏºÚÁÏ Exempt Organization Request for Updated or Replacement ST-5 form . You cannot use a P. O. Box for a physical location.

If the organization is registered for ³Ô¹ÏºÚÁÏ Employer Withholding or Sales Tax, you should use DORES to update the organization's address. Once DORES updates their information we will issue an updated ST-5.
